To start your first order, please download the "Customer Order Form","Credit Card Authorization", and "Sales Tax Resale Certificate & Credit Agreement" from the Download Forms link. Once you have all required information ready, please call, e-mail, write, or fax to us. Our minimum order and reorder is $100.00 with no quantity restriction. Drop shipment service is available with a small handling fee. We accept VISA, MASTER CARD, DISCOVER, and AMERICAN EXPRESS.
